- Your name must match the form IV certificate exactly.
- Birth certificate and date of birth must agree
- The Form IV Index Number and Form IV Certificate must be the same.
- Any of the aforementioned actions will always result in the rejection of your application.
- Make sure the Application Type and Entry Type are correct before submitting the form on the right side because doing otherwise would prevent you from starting a new application.
- After it has been registered, you won’t be able to make changes to your Form IV Index Number.
- After submitting an application, you have four (4) days to pay the application cost; else, your account will be canceled forever.
